It is rare for us to have our Physicians and Nurse Practitioners all in one place. After all, being mobile providers and responsible for delivering love, compassion and empathy into the homes of the patients we serve does not allow for many opportunities to come together and celebrate. In recognition of Christmas and the Holiday season, we were able to celebrate out Transitional Medicine team for the unique patient care they deliver and for the stories being told that highlight the critical role they play in de-fragmenting the post-acute experience for each patient/family member.
